다니던 네일샾이 문을 닫아 실업수당을 신청했는데 추가로 이런서류중 하나이상을 제출하라는 메일을 받았는데 어떻게 해야 할지 모르겠어서 여쭙니다

Dissolution/Cancellation Paperwork
Please upload a copy of any dissolution or cancellation paperwork for the employer here.
Documentation may include one or more of the following:
1. A signed statement from the corporation verifying the termination of corporate officer status.
2. Proof that the application for dissolution of the corporation has been filed with the NJ Dept of
Treasury.
3. A Certificate of Dissolution issued by the NJ
Secretary of State indicating the corporation has been dissolved.
4. Deed of assignment of assets for the benefit of creditors.
5. A copy of a petition to bankruptcy court under
Chapter 7.
