Q. I-90 Green card renewal

I got a green card in 1993 and am going to renew as the it will expire on June 2014. However I can not remember location where I applied for adjujustment status. and where the adjustment status was granted. At that time it was handled by an attorney who did not mention anything where it will be applied.

that is required in part 3 item 3 and 4 of the same forms. I called USCIS but could not get the answer.

I will be much appreciated if I can get the response.
