as u can see, as a Non-US person, For non US citizens, there are just a few fields u need to fill, namely for 1)Your name in full
2) Your country;---jut put in, N/A, as u r an individual, not a incorporation or an entity.
3) Type of owner – if you’re an individual than tick “individual”
4) Your address
6) U.S. taxpayer identification number--Put ur ITIN number here if u get one from the IRS
I am not sure but I guess u do not need the both #9) and #11) , but just for a reference;
9) Tick the box [a] for “The beneficial owner is a resident of….” and input your country’s name in the empty field. Ignore the rest (b-e), as in all likelihood you and your country has no tax treaty with the United States.
11) Tick the box for -“I have provided or will provide a statement that identifies those notational principal contracts…”
Sign at the bottom and input the date (month-day-year format).

Attach ur valid federal income tax return, perhaps either1040-nr/1040nr-ez or 1040/1040a/ez if u r a resident alien under the US Tax Code unless u qualify for an exception, and include ur original, notarized, or certified proof of identity and foreign status documents.
Because u are filing ur tax return as an attachment to ur ITIN application, u should not mail ur return to the address listed in the Form 1040, 1040A or 1040EZ instructions. Instead, send ur return, Form W-7 and proof of identity and foreign status documents to:

Also I guess,u may also apply using the services of an IRS-authorized Acceptance Agent or visit an IRS Taxpayer Assistance Center in person in lieu of mailing your information to the IRS in Austin.

For this reason u MUST file ur aW8-BEN to be provided to the IRS to prove the IRS that u are not a US citizen or a US resident alien UNDER EITHER US Tax Code or US INS Rules.
